2021 Registration information

Online registration through Formreleaf.com is open and all athletes are able to register for fall sports.

The PIAA physical packet has changed a bit. A new page regarding COVID was added and that info was also added to formreleaf. Due to this change however, the “Physical” portion is now “Section 7”. It doesn’t change much but just to make everyone aware.

Section 7, the Physical is the ONLY paper that needs to be turned in. Please email the physical to Rosenbma@npenn.org.

Online registration and the physical is due August 2nd. This gives us time to rectify any issues that could arise before all practices start.

ImPACT testing will be done at home by the athletes again this year. After an athlete registers online using formreleaf, they will get an email from ImPact Applications with a link to take the ImPact test. The link will be sent to the email used to register on Formreleaf.com. It is not immediate and can take up to 72hrs to receive the test. They don’t need to complete right away but that too will be due by August 2nd. The test should be taken in a quiet room, on a computer and not immediately following intense activity.

Once an athlete shows signs or reports signs of COVID 19 the following procedures must be followed.

1. The Athlete must stay home from practice or must be sent home immediately if showing signs.
2. The Athlete must be tested for COVID 19
a. A positive result the athlete must quarantine for 10 days
b A negative result the athlete must quarantine for 10 days, unless the athlete presents a Dr. excuse stating the athlete may return to workouts.

Athletes who state their symptoms are due to allergies must stay home from practice or be sent home if stated while at practice.

10 day quarantine – the day the athlete test positive or shows signs and symptoms is considered day four, Sitting out the next ten days meets the 14 day quarantine
